Like a good roadmap, a dental record should provide clear, comprehensive direction that speeds up your practice instead of slowing it down. This thoughtfully designed pad of forms lets you create thorough, consistent canine dental histories—especially critical to hospitals with multiple (or new-on-the-job) recordkeepers. Updated by a board-certified veterinary dentist, these two-sided records follow a format intuitive to most practices.
On the front, a large, open-mouth illustration displays the occlusal surface of numbered teeth. Adjacent is an updated, extensive legend of abbreviations and ample space for fast, shorthand notation of conditions such as periodontal pocket depths, levels of gingivitis and fractured or missing teeth, along with treatments recommended and performed. On the back, additional fields organize remarks and diagnosis, radiology evaluation and assessment, and a treatment summary and plan.
The Canine Dental Record will empower your staff to keep succinct but comprehensive records in compliance with the AAHA standard of accreditation stating that “records of dental procedures, including anatomic dental documentation or charts, are part of the medical record,” as well as the AAHA Dental Care Guidelines for Dogs and Cats, which recommend noting current and future treatment plans in medical records and addressing all abnormalities found.
Pad of 50, 8 1⁄2" x 11", two-hole punched at top, 2015, AAHA Press
“I absolutely love the variety of AAHA forms that have been designed. The different fields follow the standard of care that we strive to adhere to. The anesthesia and dental forms are easy to use and extremely thorough.” —Sue Davis Hutton, practice manager, All Pets Health Center
