Information completed on this sticker meets several of AAHA’s medical record and pain management standards.

The systems listed on the sticker are: attitude/appearance, oral cavity/teeth, mucous membranes, eyes, ears, cardiovascular, respiratory, gastrointestinal, musculoskeletal, lymph nodes, urogenital, integumentary, and nervous system. There are also blanks for: pain score, TPR, diet, body condition score, and muscle condition score.

This version includes numbers beside each system to allow you to use shorthand when elaborating on any abnormalities.
