The Home-Care Instructions Form allows you to give written instructions to clients about how to care for their pets after a procedure or surgery. Because one copy is given to the client and one copy is kept in the medical record, you’re keeping excellent records and ensuring that clients are not misinformed about care. These forms are easily customizable, helping you provide specific information to clients and improving compliance with your recommendations. This form also allows your practice to meet the AAHA standard that requires medical records to clearly reflect discharge instructions.
These forms include space for information on medications, food and water, exercise, and sutures; space for customized special instructions; and a list of problems the client should watch out for at home.
Pack of 50, 8 1⁄2" x 11", two-hole punched at top, two-part carbonless form, 2003, AAHA Press
